Greta has a greenish black thumb. Pop would say she needs a hole in the nail. He always did it with the end of his knife. Like drilling--it really releaves the pressure.

Once when my eldest son was 9 years old, he was building a stone path across a draw in our backyard and dropped the stone on his big toe. Of course, he was barefooted. he had been told numerous times to stay out of that creek...but you know boys....
Anyhow, he didn't tell me about it. That night I heard him sobbing in his bed. I asked him what was wrong and he confessed.
I called the doctor to come out to help us (In that day, doctors did those things...house calls, you know. He was babysitting though and couldn't come) but he told me to take the ice pick and heat it up white hot on the stove and then just "touch" the toenail with it and the toenail would melt and relieve the pain immediately.
I didn't think I could do that. So I went across the street and asked my neighbor to come do it. She said she couldn't do it either but she would distract him while I did it. That's what we did. I heated the ice pick white hot and she distracted him while I brought the ice pick in and touched the toenail with it. He was about to scream when he saw that ice pick but then he realized the pain was gone.
